  • APPLICATION FOR PASSPORT

  • It is advised to submit  an application for a new passport( containing biometric data) well in advance in relation to the expiry date of the held passport.

     

    A person intending to make a trip to another country ought to consider not only the date of departure, but also the regulations specifying the required validity of the passport document on crossing the border of the country( or accordingly- while applying for a visa).


    The passport document is issued upon a motion of an adult after submitting the required documents, taking the biometric data and paying a fee. The application for issuing a passport document is submitted in person.


    A person applying for issuing a passport submits:

    •  a completed application for issuing a passport or provisional passport( applications may be obtained at the consular office).

    The passport application should be completed legibly. The signature put in the box placed under the photograph must not touch or cross the lines of the box( the signature will be placed in the new passport).

    • a colour photograph of the 35x45 mm measurements, taken within the last six months against the light uniform background, reproducing the natural complexion, being in focus, and showing the eyes clearly, especially the pupils, and also the face from the top of the head to the upper part of the shoulders so that the face covers 70-80% of the photograph; the photograph is supposed  to show the person in a frontal position without a hat or dark glasses, looking straight ahead with their eyes opened, hair uncovered, with natural facial expression and closed mouth.

    A person holding a disability certificate or a certificate specifying the extent of disability( issued according to the regulations on the certification of disability or extent of disability) and also a person whose state of health does not allow to have the above-mention photograph taken in terms of the image, can submit a 35x45 mm photograph, taken within the last six months against the light uniform background, reproducing the natural complexion, being in focus, and showing the face  from the top of the head to the upper part of the shoulders so that the face covers 70-80% of the photograph; the photograph is supposed to show the person in a frontal position.


    A person with an inborn or inherited sight defect may present a photograph showing them with dark glasses. In such a case you should submit a certificate specifying the extent of disability caused by an inborn or inherited sight defect issued according to the regulations on the certification of disability or extent of disability.


    A person wearing a hat according to the rules of their faith may present a photograph showing her with a hat, which must not cover or distort the oval of the face. In such a case you should submit a certificate stating the membership of a religious community registered in the Republic of Poland.

    • an abbreviated or unabridged copy of a Polish birth certificate- in case of applying for a passport document abroad by a non-holder of PESEL number.
    • an abbreviated or unabridged copy of a Polish marriage certificate- in case of applying for a passport document after every change of surname resulting from marrying abroad.
    • a document confirming the right to have a concession or a document entitling to fee exemption.
    • possibly other documents, e.g. document containing a photograph confirming a person’s identity.

    If the data contained in the documents submitted by a person or the data contained in the available records raise doubts about the person’s identity or citizenship or there are discrepancies in the person’s data in the available documents and records, the passport organ may demand submitting other documents, particularly abbreviated copies of Polish marriage certificates, unabridged copies of Polish marriage certificates or documents confirming having Polish citizenship issued on the grounds  of the regulations of the Citizenship Act of 2nd April 2009(the Journal of Laws of 2012, item 161).


    On submitting an application it is necessary to show the currently held passport document . In case of the loss or damage of the passport document it is necessary to submit the notification of the loss or damage  of the passport document( forms are available at the consular office).


    A person applying for a passport has the fingerprints of their forefingers of both the hands taken by means of an electronic device for fingerprinting.


    In case of the unclear lines of the fingerprints  or lack of one of these fingers, the fingerprints of the pairs of middle fingers, ring fingers or thumbs are taken successively.


    The biometric data in the form of fingerprints are subject to encoding after entering the passport records.


    Attention! If need be( e.g. in a situation when the held passport document  is already invalid or it expires soon, and it is necessary to depart urgently) it is also possible to submit an application for issuing a provisional passport  for the period  of waiting  for the delivery of the passport(with biometric data).
